I UNDERSTAND AND AGREE THAT PARTICIPATION IN ANY SCIENCE ACTIVITY PROGRAM SUCH AS ADVERUTRES OF AGNAT'N'ME CARRIES A CERTAIN AMOUNT OF RISK OF ACCIDENTAL INJURY. I, THE PARENT/LEGAL GUARDIAN, OF THE ABOVE CHILD, WAIVE AND RELEASE CREATING CONSCIOUS KIDS LLC FROM CLAIMS FOR DAMAGES AND INJURY. I HAVE READ AND FULLY UNDERSTAND THE INFORMATION ABOVE AND AGREE TO ENROLL MY CHILD IN ADVENTURES OF AGNAT'N'ME CHILDREN'S ANATOMY PROGRAM.
I agree payment of $60.00 will be made by the first lesson of each month. If payment has not been recieved by the 5th, a statement will be sent including a $10.00 late fee.
Adventures of AGnat'N'Me staff has permission to photograph my child during class and reproduce prints for future advertisements and/or purchase by parent.
Payment Options:
1. Payments through Paypal are the easiest and most secure payment method and seems to be the more convenient for parents. You DO NOT need a paypal account to make a payment through paypal, all you need is a major credit card.
2. For all families whose child is attending the program at their school, and who do not wish to pay online, payment can be made to the school’s Administrator or Director unless otherwise noted.
For classes held at Independant Locations, payment can be made directly to the Adventures of AGnat’N’Me staff conducting the class.
